Release of Liability & Agreement
Both waivers must be completed and returned before any performer may take the stage. Signed waivers can be emailed to the Entertainment Coordinator or submitted in person at the event check-in table.
Submission Acknowledgment
By submitting this form, the artist or authorized representative confirms that all information provided is accurate and complete to the best of their knowledge. Submission of this form does not constitute a confirmed booking or binding agreement.
Review Process
This registration will be reviewed by the Lathrop Juneteenth Heritage Committee Entertainment Coordinator. Artists selected for further consideration will be contacted directly to discuss terms, fees, and contract details. The LJHC reserves the right to make final booking decisions at its sole discretion.
Booking Deadline
All entertainment must be fully contracted and confirmed by May 1, 2027. Submissions received after April 1, 2027 may not be considered for the 2027 celebration.
Content Standards
All performances must be appropriate for a family-friendly community event. The LJHC reserves the right to review and approve performance content prior to the event date. Content that conflicts with the values and mission of the Lathrop Juneteenth Heritage Celebration will not be permitted.
Media Rights
Unless otherwise negotiated in writing, the LJHC may photograph, record, and use images and footage of performances for non-commercial community and historical archival purposes.
